Manufacturer Narrative
A medtronic representative went to the site to test the equipment. The representative reported that a loose cable on the navigation system was reconnected to the endoscope tower and that the tower inputs were adjusted. The hardware, software, and instruments passed the system checkout. The system was found to be fully functional. The software investigation found that the reported event was unrelated to a software issue as a medtronic representative reported that the issue was with the tower. The software functioned as designed.

Event Description
A medtronic representative reported that, while in a functional endoscopic sinus surgery (fess), the video feed for the endoscope was not functioning. It was noted that the see views could be seen on the endoscope tower. The video cable was re-seated on the tower and video was restored. The output from the navigation system then did not display on the tower, stating there was no input. The surgeon opted to complete the procedure without the use of the navigation system. There was no reported delay to the procedure due to this issue. There was no impact on patient outcome. No additional information was provided.
