Investigation ¿ evaluation: a review of the complaint history, device history record, instructions for use (ifu), manufacturing instructions, quality control, and a visual inspection of the returned device were conducted during the investigation. The visual inspection of the returned device confirmed that the device was bent in several locations; bends were noted 25.5 cm and 40.5 cm from the proximal end.Additionally, a 3.9 cm section of tubing was damaged 4 mm from the distal tip.Three sections were noted to have a damaged area in the shape of an oval. The first and the second oval shaped areas of damage measured 1 mm in diameter, and were located 2.7 cm and 2.0 cm from the distal tip, respectively. The third damage was 2 mm in diameter, and was located 1.7 cm from the distal tip.Delamination was noted in the sheath interior at 11.0 cm and 31.0 cm from the proximal end.A 4.7 cm section of sheath was exiting proximally through the check flo valve.Additionally, a document based investigation evaluation was performed. There is no evidence to suggest the product was not made to specifications.A review of the device history record showed no nonconforming events which could contribute to this failure mode.It should be noted there were no other reported complaints for this lot number.Based on the information provided, the examination of the returned product, and the results of our investigation, a definitive root cause could not be determined.Measures are being conducted to address this failure mode.We will continue to monitor for similar complaints.Per the quality engineering risk assessment, no further action is required.[(b)(4)_uf medwatch_(b)(4).Pdf].
