Event or Problem Description
It was reported that during a carotid stenting procedure, the flexor shuttle tibial guiding sheath was removed over a wire without a dilator and became completely uncoiled. The sheath separated in half with the coiling exposed. The distal portion of the sheath remained within the abdominal aorta and iliac, and was required to be surgically removed. It was noted that the patient's vessels were likely calcified.Access was gained from the patient's groin; a wire guide was utilized through the sheath during the event. The following devices (unspecified manufacturer) were also inserted into the lumen of the sheath prior to removal: stents, balloon, and possibly a carotid filter.According to the initial reporter, the patient did not experience any adverse effects due to this occurrence.

Additional Manufacturer Narrative
Investigation ¿ evaluation.A review of the dimensional verification, complaint history, device history record, documentation, drawing, instructions for use (ifu), quality control, and a visual inspection of the returned device were conducted during the investigation. The visual inspection of the returned device confirmed that the sheath tubing was separated into 3 segments.Altogether, the measured length of the returned tubing was 54.5 cm.The most distal portion of the device was not returned. The first segment (proximal) measured 39.5 cm in length, and the second segment measured 15.0 cm in length.Exposed coil was exiting the second segment.Pieces of separated sheath liner were found to be embedded within the exposed coil. The proximal tubing segment contained multiple kinks/flattened portions of tubing, and appeared to be stretched. The second tubing segment also had flattened portions of tubing, and appeared to be stretched.Additionally, a document based investigation evaluation was performed. There is no evidence to suggest the product was not made to specifications.A review of the device history record showed no nonconforming events which could contribute to this failure mode.It should be noted there were no other reported complaints for this lot number.Based on the information provided, the examination of the returned product, and the results of our investigation, a definitive root cause could not be determined.Measures are being conducted to address this failure mode.We will continue to monitor for similar complaints.Per the quality engineering risk assessment, no further action is required.
