Additional Manufacturer Narrative
Exemption number e2015012.B.Braun medical inc (importer) is submitting this report on behalf of b.Braun avitum ag (manufacturer). This report has been identified as b.Braun avitum internal report # (b)(4).It was not known if there may have been something with the blood lines that may have caused the foam in the venous chamber. There was no manipulation of the extracorporeal circuit shortly before the dialysis machine triggered the venous and arterial alarms. The saftey air detect (sad) alarm did not trigger. The customer trained technician confirmed the proper operation and function of the venous and arterial alarms and the proper operation of the safety air detect. The investigation is on-going at this time.A follow-up report will be submitted when the investigation results become available.

Event or Problem Description
As reported by the user facility: reported the machine was alarming at the end of a therapy with venous and arterial pressure alarms and air was noticed going to the patient. The user facility also stated the air looked like foam. The customer also stated it may be because of the bloodlines and clamps. The patient was sent to the emergency room as a precaution only. She was not exhibiting any signs or symptoms that warranted an emergency room visit. The patient was okay and was discharged the same day to return to the facility.
