Manufacturing review: the device history records have been reviewed and this lot met all release criteria. There was nothing found to indicate there was a manufacturing related cause for this event. This is the only complaint reported to date for this lot number and failure mode.Investigation summary: the device and seven electronic photos were provided for evaluation.A visual inspection found the pebax to be peeled from the device. However, no fiber disturbance was noted to the balloon. Therefore, the investigation is unconfirmed for the reported unraveled fiber material, and confirmed for peeled pebax. It is likely that the user perceived the peeled pebax as unraveled fiber material.Per the reported event details, the user inflated the balloon approximately 16 times. Therefore, it is possible procedural issues contributed to the identified issue. The definitive root cause for the identified peeled pebax could not be determined based upon available information.Labeling review: the review of the ifu (instructions for use), indications, warnings, precautions, cautions, possible complications, and contraindications showed that the product labeling is adequate.
