Event or Problem Description
It was reported that during an inspection of field equipment on (b)(6) 2018, it was noted that two 3.5mm/4.0mm stardrive self retaining screw driver tips were twisted and warped as though they had been used to overtighten a screw when last used.It is unknown when or where the device damage occurred. It was confirmed that the device did not malfunction during a procedure and did not impact any surgery or harm any patient. This is report 2 of 2 for (b)(4).

Additional Manufacturer Narrative
Depuy synthes is submitting this report pursuant to the provisions of 21 cfr, part 803.This report may be based on information which depuy synthes has not been able to investigate or verify prior to the required reporting date. This report does not reflect a conclusion by fda, depuy synthes or its employees that the report constitutes an admission that the device, depuy synthes, or its employees caused or contributed to the potential event described in this report.Dhr review was completed.Part number: sd388.032 , synthes lot number: 6731441 , supplier lot number: n/a , release to warehouse date: 26-oct-2011 , expiration date: n/a , manufactured by synthes brandywine. No ncrs were generated during production.Review of the device history record showed that there were no issues during the manufacture of this product, and any subcomponents, which would contribute to this complaint condition.Customer quality conducted an investigation of the returned device. This complaint is confirmed. The distal stardrive tip on the returned 6+ year old device shows post manufacturing damage in the form of a twisted tip.The tip is twisted approximately 15 degrees in the direction of resistance met during attempted screw insertion.Whether this complaint can be replicated at customer quality (cq) via functional test is not applicable for this complaint condition. No new malfunctions were identified as a result of the investigation.A relevant dimensional analysis was unable to be performed due to post manufacturing damage.Product drawing was reviewed during this investigation. No product design issues or discrepancies were observed.Review of the device history record showed that there were no issues during the manufacture of this product, and any subcomponents, which would contribute to this complaint condition. The material and relevant material properties including material hardness were determined to be conforming at the time of manufacture based on review of the dhr.A definitive root cause for this complaint was unable to be determined. It is most likely that the returned 6+ year old reusable instrument was subjected to an insertion torque exceeding its capability which led to its deformation.Based on the investigation findings, it has been determined that no corrective and/or preventative action is proposed.Device was used for treatment, not diagnosis. If information is obtained that was not available for the initial medwatch, a follow-up medwatch will be filed as appropriate.
