Device evaluated by mfr: promus premier us mr 4.00 x 32mm stent delivery system was returned for analysis.A visual examination of the crimped stent identified damage. The mid-section of the stent was damaged and bunched causing the proximal end of the stent to move on the balloon 2mm distally. The distal end of the stent was still in the crimped position and was within max crimped stent profile measurement. The balloon cones were reviewed and no issues were noted. The balloon wings were tightly wrapped and evenly folded and were not subjected to positive pressure.A visual and tactile examination of the device revealed no issues with the hypotube. An examination of the shaft polymer extrusion revealed damage to the inner. The inner shaft polymer extrusion was bunched 3mm distal from the bi-component bond. The tip was visually examined and no issues were noted. No other issues were identified during the product analysis. The investigation conclusion is caused by other device as another device/drug/subsequent procedure caused the complaint event.(b)(4).
