Manufacturing review: the device history records were reviewed with special attention to the raw materials, subassemblies, manufacturing process and quality control testing. This lot met all release criteria. There was nothing found to indicate there was a manufacturing related cause for this event. This is the only complaint reported to date for this lot number and failure mode.Investigation summary: the device was returned for evaluation.A visual inspection found the outer fiber-based jacket to have been completely inverted and detached from the balloons. The jacket was returned over the catheter shaft.Additionally, the outer parylene coating on the balloons was found to be peeling throughout the length of each balloon. Therefore, the investigation is confirmed for a detached outer jacket, and for peeled parylene. However, the investigation is inconclusive for the alleged unintended movement as the conditions of use could not be recreated. It is likely that the detachment of the outer jacket contributed to the identified peeled parylene. However, the definitive root cause for the identified detachment of the outer jacket or reported unintended balloon movement could not be determined based on the available information.Labeling review: the review of the ifu (instructions for use), indications, warnings, precautions, cautions, possible complications, and contraindications showed that the product labeling is adequate.
