The information provided by bard represents all of the known information at this time.Despite good faith efforts to obtain additional information, the complainant / reporter was unable or unwilling to provide any further patient, product, or procedural details to bard.The following were reviewed as part of this investigation: patient severity, frequency analysis, applicable previous investigation(s), sample (if available), applicable fmea documents, labeling, and applicable manufacture records.Based on a review of this information, the following was concluded: the complaint of the extension tubing detaching from the hub is confirmed but the exact cause is unknown.The product implicated was a powerglide 8cm.The sample returned consisted of a male hub extension set connector.Use residue was visible throughout the hub.The extension tubing was not returned with the connector.As the extension set tubing was not returned attached to the hub, the complaint is confirmed but the cause is unknown.Based on the description of the reported event and condition of the returned sample, possible contributing factors include excessive tensile force during use.No further action is required because the reported event could not be related to a deficiency in product manufacture or deficiency while under correct product use.A lot history review (lhr) review is not possible, as no manufacturing lot number has been provided by the complainant.
 
Event Description
It was reported that after the device was placed, prior to the first dressing change the nurse accessed the line and the catheter detached from the hub.Device was replaced, no injury to the patient.
