Additional Manufacturer Narrative
|
|
The device history records have been reviewed and this lot met all release criteria. There was nothing found to indicate there was a manufacturing related cause for this event. This is the only complaint reported to date for this lot number and failure mode. The device was returned for evaluation.A visual inspection found the device returned stuck in the users introducer sheath. The distal end of the sheath was flared, and the balloon material was prolapsed over the distal tip. Therefore, the investigation is confirmed for the reported retraction issues.Additionally, a complete break at the proximal balloon butt joint was identified; the inner guidewire lumen was intact. Therefore, the investigation is confirmed for a break. It is likely that the retraction issues led to the catheter break at the butt joint. However, the definitive root cause for the retraction issues could not be determined based upon the available information. The review of the ifu (instructions for use), indications, warnings, precautions, cautions, possible complications, and contraindications showed that the product labeling is adequate.(b)(4).
