Additional Manufacturer Narrative

Manufacturing review: the device history records have been reviewed and this lot met all release criteria. There was nothing found to indicate there was a manufacturing related cause for this event.Investigation summary: the device was returned for evaluation.A visual inspection found the device returned detached in multiple locations.A total of five device segments were returned, making up the entire device. The balloon was also noted to be detached at the proximal balloon weld joint. Therefore, the investigation is confirmed for both catheter and balloon detachments. The original sheath used with the device was not returned, and the device was unable to be functionally tested due to the returned sample condition. Therefore, the investigation is inconclusive for the reported balloon rupture, and sheath-related retraction issues. It is possible that retraction issues could have led to the identified catheter detachments. However, the definitive root cause for the identified detachments or reported rupture and sheath-related retraction issue could not be determined based upon available information. It is unknown whether procedural issues or the original sheath used in the procedure contributed to the event.Labeling review: the review of the ifu (instructions for use), indications, warnings, precautions, cautions, possible complications, and contraindications showed that the product labeling is adequate.
