Additional Manufacturer Narrative
|
|
At the completion of the clinical evaluation, based on the information received there were substantial evidence to support the reported persistent type ia endoleak. There was evidence to suggest that the endoleak type ia was a persistent leak at implant that was not included in the event as reported. This was discovered on (b)(6) 2018 during review of the implant angiogram on (b)(6) 2016.The most likely cause of the endoleak type ia is device related.Additionally there was evidence to reasonably support and endoleak type ib.The most likely cause of the type ib leak is anatomy related.Procedure related harms for this complaint could not be determined. The final patient status was reported to be doing well and under surveillance due to a persistent endoleak post repair.Devices remain implanted in the patient and were not returned, no evaluation completed. The review of manufacturing lot confirmed all devices met specifications prior to release. This complaint is not capa eligible at this time. These types of events will be monitored and trended as part of the quality system.Correction: (b)(4).
