It was reported that during an unspecified procedure, a 6f x 30 cm flexor introducer sheath fractured upon insertion in the patient's aortoiliac bifurcation, leaving a 6 cm piece inside the patient's anatomy. An up and over approach was being utilized. The broken sheath was removed, and replaced with another manufacturer's sheath to contain hemostasis.Patient was then intubated in case of possible open cut down to retrieve the foreign body.Once the patient was intubated, the physician accessed the right external iliac artery using ultrasound guidance.Another manufacturer's 8f x 11 cm sheath was then placed in the artery, followed by using another manufacturer's endovascular snare to retrieve the broken off piece of sheath. The percutaneous foreign body removal was successful and an open cutdown was unnecessary. An angiogram was then performed to check for any remaining foreign bodies, after confirming all foreign bodies had been removed, the right sided 8f sheath was removed and another manufacturer's 6 mm x 2.5 cm stent was placed over the access hole in the right external iliac artery.A section of the device did not remain inside the patient¿s body. The patient did not require any additional procedures due to this occurrence.According to the initial reporter, the patient did not experience any additional adverse effects due to this occurrence.
