The production device history record (dhr) for this intra-aortic balloon pump (iabp) was not required to be reviewed per the company standard operating procedure since the device manufacture date is greater than one year from the event date.A getinge field service engineer (fse) evaluated the iabp and was unable to reproduce the reported "low vacuum alarm" issue.However the fse found that the safety disk was due for replacement and he replaced it.The fse then completed preventative maintenance (pm) and the iabp was cleared for clinical use.
 
Event Description
It was reported that while in use on a patient the cs300 intra-aortic balloon pump (iabp) was alarming low vacuum intermittently and would self-correct then alarm again.There was no patient harm and no adverse event was reported.The company rep verified that the nurse had checked all connections, and then explained that it could be related to the safety disk possibly loosening up on the back of the pump.The company rep directed the nurse to the safety disk¿s location and asked if it was tight.The nurse stated that it was.The company rep recommended that the iabp be switched out for a different one and sent for evaluation.
