The manufacturing and material records for the mitroflow bioprosthetic pericardial heart valve, model # dla25, s/n (b)(4), , as they pertain to the reported event, were retrieved and reviewed by quality engineering at livanova (b)(4) corp. The results confirmed that this valve satisfied all material, visual, and performance standards required for a dla25 mitroflow aortic pericardial heart valve at the time of manufacture and release. The manufacturer was unable to obtain additional information about this event, despite attempts to follow up with the involved physician. This event was reported in a conservative manner due to the limited information available.Based on the information provided in the registry, the patient's death was attributed to "proximate cardiac cause".Furthermore, the patient's complicated clinical history (section 12) suggests that patient factors likely contributed to this event. As such, there is no clear indication that the event was device-related.No manufacturing deficits were identified in the document review performed. However, given the limited information available, the root cause of the event cannot be determined.Should further information be received in the future, the case will be revisited and appropriate investigative actions will be taken.
