Event Description
Per vascular access nurse: today ((b)(6) 2018) pulled a picc back 5cm as patient was turned and went into sustained v-tach.Picc was placed (b)(6) 2018 via the new sherlock.The icu md reviewed latest cxr ((b)(6) 2018) to see if it was the picc.The cxr film and annotation: "picc in right atrium." since the cxr was not ordered for picc confirmation "our" dept was not notified.The picc did not migrate.The original depth was 0cm and today it was still at 0cm prior to pulling it back.The tracing in the patient's chart from picc placement: very tall tented p waves w/o noticeable deflection.
