Additional Manufacturer Narrative
The reported event that knife light 10/pk was alleged of 'breakage during surgery' could be confirmed.Based on investigation, the root cause was attributed to be user related. The failure was caused by inappropriate use of the device. The device inspection revealed the following: damages of the polymer tip are clearly evident, resulting that the blade detached slightly from the polymer structure. This was caused by the application of excessive force on the tip of the knife. This would probably be caused by using of the knifelight as a retracting device to pry or separate muscle or ligaments, which is a clear contradiction to what is recommended in the instruction for use.A review of the device history for the reported lot did not indicate any abnormalities. No corrective actions are required at this time.A review of the labeling did not indicate any abnormalities. No indications of material, manufacturing or design related problems were found during the investigation. If any further information is provided, the investigation report will be updated.
