The impacted product was received by the failure analysis lab on september 3, 3030.The investigation of the returned device was completed by the failure analysis lab on september 23, 2020.Device evaluation summary: during the visual evaluation, an anomaly was observed on the anterior extending to the posterior view consistent with a crease/fold.A tear on the anterior aspect measuring approximately 0.2 cm was also observed within the crease/fold.The evaluation determined that the loss of shell integrity is consistent with a crease fold deflation of the implant shell, which is a known inherent risk of saline-filled mammary prosthesis.Deflation can occur at any time after implantation, but it is more likely to occur the longer the implant is implanted.Creating wrinkles or folds should be avoided during implantation or other procedures as it can result in deflation at a later time.Breast implants may also simply wear out over time. Each device is visually inspected during manufacturing to ensure the device meets the required specifications prior to shipment.Manufacturer¿s reference number: (b)(4).

Additional Manufacturer Narrative
Additional information was received on (b)(6) 2020.In addition to the left sided deflation reported initially, right sided capsular contracture was also noted. When the devices were explanted, bilateral prosthesis replacement with 500cc mentor memorygel breast implants was performed. This report relates to the left prosthesis.See 1645337-2020-13284, for contralateral prosthesis report.Manufacturer¿s reference number: (b)(4).
