Additional Manufacturer Narrative
A wallstent biliary delivery system was received for analysis; the stent was not returned.Visual examination of the returned device found that the stainless steel tube broken and kink. The clear exterior tube at the distal end and the inner member were found kinked. No other issues with the device were noted. The noted damages on the returned device were consistent with the excessive force being applied to the delivery system. The investigation concluded that procedural factors as the handling of the device and normal procedural difficulties encountered during the procedure could have possibly affected the device performance and its integrity contributing to the failure experienced by the customer. Therefore, the most probable root cause is operational context.A labeling review was performed and from the information available, this device was used per the directions for use (dfu)/product label.A review of the device history record (dhr) was performed and confirmed that this device met all material, assembly and performance specifications at the time of release for distribution.A search of the complaint database confirmed that no other similar complaints exist for the specified lot.
