Additional Manufacturer Narrative
An ultraflex esophageal ng proximal release covered stent and delivery system were returned for analysis. The stent was received partially deployed. The stylet was still inserted at the distal end.Visual examination of the returned device found the shaft was kinked near the handle.Functional evaluation revealed that it was possible to deploy the stent with no difficulty by pulling the finger ring. The stent was measured to be within specifications. No other issues were identified with the stent and delivery system. The damage noted with the delivery system is likely due to handling /manipulation of the device during the procedure. The investigation concluded that the observed failure and the reported event were likely due to anatomical or procedural factors encountered during the procedure which limited the performance of the device. Therefore, the most probable root cause is operational context.A labeling review was performed, and from the information available this device was used per the directions for use (dfu)/product label.A review of the device history record (dhr) was performed and confirmed that this device met all material, assembly and performance specifications at the time of release for distribution.A search of the complaint database confirmed that no other similar complaints exist for the specified lot.
