Additional Manufacturer Narrative
|
|
The device history records have been reviewed and this lot met all release criteria. There was nothing found to indicate there was a manufacturing related cause for this event. This is the only complaint reported to date for this lot number and failure mode. One partial flair guidewire lumen was returned for evaluation.Visual evaluation identified a detachment of the inner lumen and atraumatic tip.Functional testing was not viable. The investigation is confirmed for detachment, however, removal difficulty and guidewire incompatibility are inconclusive due to sample condition. The reported break is unconfirmed as the inner lumen and distal tip were actually detached from the system. The inner lumen likely detached due to the tension seen during removal issues. However, the root cause for removal issue could not be determined based upon available information. It is unknown whether patient and/or procedural factors contributed to the event. The review of the ifu (instructions for use), indications, warnings, precautions, cautions, possible complications, and contraindications showed that the product labeling is adequate.(b)(4).
