Additional Manufacturer Narrative
(b)(4).During processing of this complaint, attempts were made to obtain complete event, patient and device information.Evaluation summary: visual and dimensional inspections were performed on the returned device. The outer member was bunched and torn. The resistance with the guide wire was unable to be confirmed due to the condition of the returned device.A review of the lot history record revealed no manufacturing nonconformities that would have contributed to this complaint.Additionally, a review of the complaint history of the reported lot revealed no other incidents. The reported difficulties appear to be due to case circumstances. It is likely that during use the clearance between the inner member of the catheter and the guide wire became reduced causing the catheter to become frozen over the guide wire.Continued attempts to remove the catheter likely caused the noted shaft damage. There is no indication of a product quality issue with respect to the design, manufacture, or labeling of the device.
