Additional Manufacturer Narrative

(b)(4).It was noted that the expansion arms sub assembly was loose and was able to shift position/angle without input.Visual examination revealed that the expansion arms sub assembly was missing a screw which holds the arms of the sub assembly together. The screw was not returned to customer quality unit. There is no evidence of any breakage on the device. The expansion knob mechanism, release lever and pistol grip were functioning as intended. The gold sleeve which allowed the handles rotate and lock in at 90 degree intervals was functioning as intended.A review of the device history record was conducted. No issues were identified during the manufacturing and release of this product that could have contributed to the problem reported by the customer. All complaint trends will be evaluated as a part of the depuy spine monthly complaint review meeting.A definitive root cause for missing a screw which holds the arms of the sub assembly together cannot be positively determined with the information provided. However, noted damage suggests that the inserter/expande was likely subjected to unanticipated overloading forces while expanding the cage to the desired height, resulting in a screw which holds the arms of the sub assembly together falling apart. As there has been no issue identified in the manufacturing or release of the device that could have contributed to the problem reported by the customer and no systemic trends were found, this complaint file will be closed with no further action required. If information is obtained that was not available for the initial medwatch, a follow-up medwatch will be filed as appropriate.
