Event Description
|
The patient was undergoing a thrombectomy procedure using a penumbra system aspiration pump max 110v (pump max).During the procedure, a pump max lost all aspiration power and was making a small purring noise; therefore, it was disconnected.The procedure was completed using another pump max.It was reported that after turning off the pump max and waiting awhile, the pump max started working again but would only reach -20 to -22 mmhg.There was no report of an adverse effect to the patient.

Results: the pump max was powered on and generated a vacuum pressure of approximately -28.5 inhg.The pump was run continuously for 15 minutes without issue.Conclusions: evaluation of the returned pump max revealed a fully functional device.The pump max was powered on and generated a vacuum pressure within specification.The pump was run continuously for 15 minutes without issue.The reported noise and lost vacuum pressure was unable to be confirmed.Penumbra pumps are visually inspected and functionally tested during incoming quality inspection.The manufacturing records for this lot were reviewed and did not reveal any outstanding discrepancies, design, or quality concerns.
