Additional Manufacturer Narrative
An internal investigation was performed for a false negative result associated with the bact/alert® fa blood culture bottle (reference 259791).The patient was on chemotherapy, but no further information was provided. The customer declined to provide a virtuo backup that includes the bottle readings. Two graphs for each bottle type were reviewed but it was not possible to thoroughly evaluate the data because raw readings were not provided.A virtuo screenshot showed bottle type fa plus bottle as negative and the bottle type fn plus bottle as positive. The manufacture records review could not be performed as the customer did not provide the lot number for the bact/alert® fa plus bottle type. The bact/alert® fa plus instructions for use (ifu), 9312050 e -2016-04 was reviewed showing it contains sufficient information regarding procedures and precautions. No bottles were requested to be returned as a result of this investigation as the customer sub-cultured the inoculated bottles.Visual inspection was not performed on retained bottle samples since the complaint was not about visual aspects of the product.Insufficient information was provided, potential causes of the false negative result in the bact/alert® fa plus bottle are delayed entry, insufficient sample volume, and antimicrobial therapy. There were no other false negative or sample related complaints associated with bact/alert® fa plus bottle type.A historical review of the last year's data found no other similar complaints.
