Description of event according to short form complaint filed: it is alleged that "[pt] received a gunther tulip filter on (b)(6) 2012".Patient outcome: it is alleged that [pt] was injured without further explanation.Hospital and medical records have been requested but not yet provided.

Patient allegedly received an implant on (b)(6) 2012 via the right common femoral vein due to contraindication to anticoagulation and a risk of deep vein thrombosis post trauma.Patient alleges migration, tilt, organ perforation, pulmonary embolism.Patient further alleges heaving breathing, shortness of breathe, weakness, disorientation, heart attack caused by two large blood clots.

Additional information received.The patient alleges that the filter was successfully removed on (b)(6) 2019 at the patient's request.Patient is alleging pulmonary embolism, vena cava perforation, tilt, and blood clots.The patient is also alleging that they cannot walk or stand for long periods of time.Need help walking stairs.Running or strenuous activity is an impossibility.Depression.
