It was reported that the patient presented in the emergency room after experiencing a shock from their implantable cardioverter defibrillator experiencing dizziness.Upon interrogation, the patient was in ventricular tachycardia (vt) and had received high voltage therapies.Later that evening, the patient was cardioverted with an external shock.Upon review, the egm for a vt episode was missing.On (b)(6) 2018, the physician elected to perform a defibrillation threshold test.During the test, there was an instance where the patient¿s induced vt rhythm was mistaken as noise, withholding therapy.Additionally, fluoroscopy indicated that the right ventricular lead exhibited an extrusion.Options were discussed and programming changes were made.The patient was doing well following the events.
