(b)(4).During processing of this complaint, attempts were made to obtain complete event, patient and device information. The incident information was reviewed; however, the device was not returned for analysis.A review of the lot history record identified no manufacturing nonconformities issued to the reported lot that would have contributed to this event.Additionally, a review of the complaint history did not indicate a lot specific quality issue. The investigation determined the reported difficulties appear to be related to circumstances of the procedure as it is likely the device interacted with the guide wire due to a build-up of procedural contaminates on the guide wire resulting in the reported difficulty to advance and remove the sds.Additionally, the interaction with the guide wire and subsequent difficulties caused the additional identified damage to the device. There is no indication of a product quality issue with respect to manufacture, design or labeling.

Subsequent to the initial/final filed report, the following information was received: a user facility medwatch report was received with event status serious injury, required intervention and reported "while md was deploying a cardiac stent, the stent would not slide off the guidewire and began to bunch up on the wire. The md was able to slide it back on the guidewire and removed it without damage to the patient." follow-up with the account confirmed that the stent delivery system (sds) shaft was inadvertently reported as being bunched when it was in fact the stent that had bunched; however, the wire, stent and sds were removed together as a single unit requiring no further intervention as initially reported. There were no reported adverse patient effects. No additional information was provided.
