Report confirmed.Evaluation determined that the two pieces of the t12mh20 tool was returned inside the tt12c telescoping attachment (lot number 064606-03).The one piece of the tool could not be removed from the attachment. It was noted that the tool had heat related discoloration present on tube exit and tool.Fracture site is consistent with location of distal bearings in tt12c telescoping tube and the discoloration on tool is indicative of heat. The likely cause was identified that the distal bearings no longer supports the tool effectively. The tool contacted the non-rotating diameter of the tube exit and the excessive heat caused the tool to fracture. The user manual contains the following warning ¿do not use excessive pressure, such as bending or prying, on attachments or dissecting tools. This may cause tool to bend or break and cause injury to patient, operator and/or operating room staff.¿ we will continue to track and trend this complaint type.If information is provided in the future, a supplemental report will be issued.
