The associated complaint devices were not returned. The clinical/medical team concluded, it was reported that right hip revision surgery was performed due to a fracture of the femoral stem. The revision operative report noted there was nothing to indicate what could have caused the fracture. The explants were sent to evaluation, but the results were not provided. Conclusion: it was noted during revision that there was nothing to indicate what could have caused the fracture of the femoral stem. The explanted stem was reported to have been sent to for evaluation; however, no results were provided for review.Without the supporting lab/pathology results, imaging, and/or the analysis of the explanted components, the root cause of the reported fractured femoral stem cannot be confirmed and it cannot be concluded that the reported event was associated with a mal-performance of the implant. The patient impact beyond the pain, revision, and expected transient post-op convalescence period cannot be determined. No further clinical assessment is warranted at this time.A review of the production documentation for the corresponding products did not reveal any deviation from the standard manufacturing processes.A review of complaint history for the listed parts revealed no prior complaints for the listed batches/failure mode.Without the actual product involved, our investigation cannot proceed. If the device or new information is received in the future, this complaint can be re-opened.We consider this investigation closed.
