Added medical history. Conclusion code remains unchanged.Added medical record information.Additional details regarding the patient's clinical course were ascertained from a review of medical records and are as follows: operative records dated (b)(6) 2017 state the patient underwent ¿1.Right breast reconstruction with immediate abdominally-based perforator free flap for breast reconstruction.2.Reconstruction of right hemi-abdominal wall with implantation of retrofacial biosynthetic mesh.3.Application of waistline incisional negative pressure dressing 4.Spy perfusion assessment of mastectomy flap and free flap.¿ postoperative diagnosis states: ¿right breast invasive lobular carcinoma.¿ the (b)(6) 2017 records state the patient "is a 59-year-old woman who was recently diagnosed with an invasive lobular carcinoma of the right breast. After being fully counseled on options by dr.(b)(6), she presented to me for counseling on reconstructive options. She chose a non-nipple-sparing mastectomy for her right breast and after being counseled on the various and sundry options, elected to proceed with right-only breast surgery, and for reconstruction requested an immediate diep flap, which i sanctioned.¿ the (b)(6) 2017 operative report states regarding the breast reconstruction: ¿there was no accompanying artery suitable for microsurgery, so we continued with a deeper dissection and followed our plan which was initially to raise a right hemiabdominal flap to the right breast, preserving, if needed, the left hemi-abdomen as a pedicle tram.We carried the dissection from the point in the flank and raised the skin and fat off of the anterior abdominal wall fascia. When we got to the level of the linea semilunaris, we slowed the dissection and went into a lower bovie setting and used, per our routine, a freer elevator to perform a blunt careful dissection along the fascia. In so doing, we identified 1 sizeable periumbilical medial lobe perforator and an adjacent lateral lobe perforator and just caudal to that on the abdominal wall, another lateral lobe perforator. We then quickly tested these and decided to sacrifice the medial lobe perforator, which was clipped and divided and then having completed the suprafascial dissection, we lined up the 2 lateral lobe perforators, entered the fascia using a 15 blade just above the superior of the 2 and then performed an intramuscular dissection which was a true perforator dissection, sparing all muscle.¿ the (b)(6) 2017 operative records continue regarding the breast reconstruction: ¿we carried this all the way down through the muscle until we got into the lateral space and then we were able to carry the dissection all the way down to the vessel's origin off the iliac. We then put vascular tape around the artery and vein. There was only 1 vein. When dr.(b)(6) was finished and we were still carrying on the flap dissection, we divided and conquered.I worked with ms.(b)(6), rn, to finish the last stages of vessel dissection in the groin and (b)(6) went to the chest and removed the 3rd rib cartilage and then a subperichondrial dissection and found and prepared the right internal mammary artery and vein.Again, in the chest, there was interestingly only 1 particularly usable vein. These were dissected circumferentially and prepared for microsurgery.¿ the (b)(6) 2017 operative report states that after the mastectomy and right breast reconstruction was performed: ¿dr.(b)(6) and (b)(6), rn, worked to repair the donor site defect in the right hemiabdominal wall. That was done by undermining the fascia by several cm in both directions and then they implanted a piece of gore synecor mesh that was reference (b)(4), s/n number was (b)(4).It was cut down to size and ultimately implanted by placing parachuted 2-0 pds horizontal mattress sutures in all directions and then closing the fascia over this with a looped 0 pds suture. They also anesthetized the abdominal wall by injecting bupivacaine in and around this repair. The abdominal donor site was then drained with two 19-blake drains brought through far posterior flank counterincisions on trocar. The drains were secured to the skin with 0 silk stitches. We asked for the waist to be flexed so that we could close the skin and the skin was closed by placing a running monoderm barbed suture through scarpa fascia and then similarly subcuticular closure was done with a barbed dermal suture.¿ the (b)(6) 2017 operative records state: ¿a counterincision was made in the advanced abdominal wall flap and the umbilicus was pulled up on a setting suture and inset and an umbilicoplasty was performed and this was done with deep 2-0 vicryl suture and then a running 5-0 fast-absorbing gut suture at the level of the skin. Before being able to close the abdominal wall, obviously the left hemi-abdominal flap was sheared off. There was no fascial entry. That was weighed and i used that to estimate the weight of the right hemi-abdominal flap which was 720 grams. At the end of the case, we cleaned and dried the skin of the waist and we applied a customizable prevena incisional negative-pressure dressing to the waistline incision, including a superior extension to cover the umbilicoplasty. The right breast was covered with a sheet of xeroform.¿ the records confirm a gore® synecor intraperitoneal biomaterial (gksc12/15358337) was implanted during the procedure.Records dated (b)(6) 2017 state the patient was seen for postoperative follow up from her (b)(6) 2017 procedure.¿she feels that she's recovered fairly well from the surgery, but was concerned about some exertional dyspnea; her pulse ox at time of arrival was 92~-0.¿ abdominal exam notes state: ¿protuberant, wearing post op support belt: scar not examined , soft, nt/nd, bs present, some guarding no rebound, no masses palpated, no hepatosplenomegaly, no hernias present.¿ ct scan records dated (b)(6) 2017 state: ¿upper abdomen: no expander reconstruction at time of r skin sparing mastectomy.Placement of the tissue expander is pending intra operative spy findings. She is a good candidate for diep flap reconstruction.May proceed with staged unilateral diep flap reconstruction well after adjuvant therapy has completed.Understanding she may need adjuvant radiation he also agreed to perform immediate unilateral diep with understanding it may need to be left larger to accommodate for shrinking after rads.He counseled pt she cannot have muscle plication during diep flap.Pt understood; all questions answered.Dr.(b)(6) reviewed differential dx, surgical techniques, chance of success with pt.Reviewed risks, benefits, alternatives, indications, answered all questions.Noted routine risks of surgery can include but are not limited to bleeding, infection, scarring pain, seroma, hematoma, dehiscence, wound complications. For expander/implant based reconstruction pts, dr.(b)(6) further noted that risk can also include peri-prosthetic infection, capsular contractures, muscle spasm, and/or pain, need for biologic mesh to support the lower pole, possible exposure or explantation of the expander for a variety of reasons like positive margins on final path, exposure, persistent pain, persistent infection despite antibiotics. For autologous free flap reconstructions (diep flaps) we noted risk includes flap failure or necrosis, flap shrinkage, thromboembolic events requiring possible anticoagulation, placement of expander as back-up operation at initial surgery, a week inpatient stay, a month of extensive recovery, 6 months no heavy lifting. We discussed donor site, possible morbidity such as hernia, bulging, fat necrosis, skin necrosis, umbilical necrosis and need for scar revisions such as diagnosed with an invasive lobular carcinoma of the right breast. After being fully counseled on options by dr.(b)(6), she presented to me for counseling on reconstructive options. She chose a non-nipple-sparing mastectomy of her right breast and after being counseled on the various and sundry options, elected to proceed with right-only breast surgery, and for reconstructions requested an immediate diep flap, which i sanctioned.¿ implant procedure: right breast reconstruction with immediate abdominally-based perforator free flap for breast reconstruction.Reconstruction of right hemi-abdominal wall with implantation of retrofacial [sic] biosynthetic mesh.Application of waistline incisional negative pressure dressing.Spy perfusion assessment of mastectomy and free flap.Implant: gore® synecor intraperitoneal biomaterial (15358337/ gkfc12) 12 cm, circle.Implant date: (b)(6), 2017 ¿ description of hernia being treated: ¿while dr.(b)(6) started on the mastectomy, my partner and i started at the abdomen.A 10 blade was used to incise both the upper and lower abdominal incisions.Along the waist, care was given to preserve the superficial vasculature coming from the groin and on the right side where we found a sizeable superficial inferior epigastric vein which we preserved for potential additional venous flow. There was no accompanying artery suitable for microsurgery, so we continued with a deeper dissection and followed our plan which was initially to raise a right hemi-abdominal flap to the right breast, preserving, if needed, the left hemi-abdomen as a pedicle tram.We carried the dissection from the point in the flank and raised the skin and fat off of the anterior abdominal wall fascia. We then quickly tested these and decided to sacrifice the medial lobe perforator, which was clipped and divided and then having completed the suprafascial dissection, we lined up the 2 lateral lobe perforators, entered the fascia using a 15 blade just above the superior of the 2 and then performed an intramuscular dissection which was true perforator dissection, sparing all muscle. We carried this all the way down through the muscle until we got into the lateral space and then we were able to carry the dissection all the way down to the vessel¿s origin off the iliac.¿ ¿while this was being done, dr.Singh and marybeth gall, rn, worked to repair the donor site defect in the right hemi-abdominal wall. That was done by undermining the fascia by several cm in both directions and then they implanted a piece of gore synecor mesh that was reference gksc12, s/n number was (b)(6).¿ ¿ implant size and fixation: ¿it was cut down to size and ultimately implanted by placing parachuted 2-0 pds horizontal mattress sutures in all directions and then closing the fascia over this with a looped 0 pds suture. They also anesthetized the abdominal wall by injecting bupivacaine in and around this repair. The abdominal donor site was then drained with two 19-blake drains brought through far posterior flank counterincisions on trocar. The drains were secured to the skin with 0 silk stitches. We asked for the waist to be flexed so that we could close the skin and the skin was closed by placing a running monoderm barbed suture through scarpa fascia and then similarly subcuticular closure was done with a barbed dermal suture.A counterincision was made in the advanced abdominal wall flap and the umbilicus was pulled up on a setting suture and inset and an umbilicoplasty was performed and this was done with deep 2-0 vicryl suture and then a running 5-0 fast absorbing gut suture at the level of the skin. Before being able to close the abdominal wall, obviously the left hemi-abdominal flap was sheared off. There was no fascial entry. That was weighed and i used that to estimate the weight if the right hemi-abdominal flap which was 720 grams. At the end of the case, we cleaned and dried the skin of the waist and we applied a customizable prevena incisional negative-pressure dressing to the waistline incision, including a superior extension to cover the umbilicoplasty.¿ ¿ post-operative period: [2 days] - (b)(6) 2017: ¿prevena dressing hospital and planned for surgery the following day.¿ explant procedure: debridement of open wound of the abdomen, including skin subcutaneous tissue fascia and removal of previously placed bio-synthetic synecor mesh.Primary closure of right rectus fascia primarily with vloc suture.Scar revision and complex closure of abdominoplasty waistline incision for 20 cm on the right flank.Explant date: (b)(6), 2017 ¿ ¿i began surgery by sharply debriding the right open abdominal wound.A scalpel was used to debride devitalized tissue in the skin and subcutaneous and fascial layers.A scalpel was used to release sutures which were previously placed to hold the biosynthetic in the retro-rectus position. The rectus fascia had separated and the mesh was obviously exposed. It was challenging but ultimately the mesh was removed in its entirety. It was sent to microbiology for culture. The peritoneal space was intact and there was no involvement with the peritoneal cavity. The wound was copiously irrigated with antimicrobial solution and then vigorously scrubbed for further debridement. The fascial edges were freshened and i was still able to pull them together primarily the and obtain primary right rectus fascial closure utilizing 0-vloc suture.Next a 19 french drain was placed in the subcutaneous plane and then the 20 cm transverse abdominoplasty scar was revised and closed utilizing complex closure technique.Scarpa¿s fascia was first closed utilizing interrupted 2-0 vicryl suture. This was followed by interrupted buried dermal 3-0 vicryl suture and absorbable insorb staples. This was followed by a running 4-0 monocryl dermis suture and a 20 cm revealing closed incision negative pressure dressing.¿ relevant medical information: ¿ (b)(6) 2017: tissue culture: ¿source: abdominal
