A visual evaluation of the device found that a clear plastic side of the pouch where the label goes placed was found to be broken, also the label was broken and the product has wrinkles indicating the manipulation of the unit.Based on the product analysis, it is possible that the way in which the device was handled and manipulated during the processes of shipping/receiving and/or storage may have contributed to the failure encountered (pouch and label broken); the defect found is typically caused due to incorrect manipulation of the product while its transport, taking into consideration that the unit was returned inside its original package.Additionally, the returned product has wrinkles indicating the manipulation of the unit. Therefore, cause traced to transport/storage is selected as the most probable root cause for the complaint.A dhr review was performed and no anomalies were observed.
