Investigation - evaluation a review of the complaint history, device history record, drawing, instructions for use (ifu), manufacturing instructions, quality control and specifications of the device was conducted during the investigation. The complaint device was not returned; therefore, no physical examinations could be performed. However, a document based investigation evaluation was performed.Sufficient controls are in place to detect this failure mode prior to device release.Product labeling attached to the wire guide holder illustrates that withdrawal and/or manipulation of the distal spring coil portion of the wire guide through the needle tip may result in breakage.A review of the device history record was conducted and revealed one related nonconformance in a scrapped device for kinks introduced during grinding. It should be noted that there were no other complaints reported for lot 8847469.Based on the information provided, no returned product and the results of our investigation, it is likely that the cause can be traced to the user and a failure to follow instructions. The customer stated that the wire guide was attempted to be withdrawn through a chiba needle and the distal end caught on the needle¿s bevel.Wire guide separation is a likely effect of withdrawing a wire guide through a needle.A label attached to the packaging of this device illustrates not to withdraw the wire guide through an introducer needle. The customer will be notified of these investigation results in a custom letter.Per the quality engineering risk assessment no further action is required.Cook will continue to monitor for similar events. This report is required by the fda under 21 cfr part 803 and is based on unconfirmed information submitted by others.Neither the submission of this report nor any statement contained herein is intended to be an admission that any cook device is defective or malfunctioned, that a death or serious injury occurred, nor that any cook device caused, contributed to, or is likely to cause or contribute to a death or serious injury if a malfunction occurred.
