Event or Problem Description

It was reported that shaft break occurred.A 4.00 x 12mm synergy ii drug-eluting stent was advanced for treatment; however, it was noted that the main body was fractured more than 15cm from the hub, resulting in two separate pieces of the stent delivery system (sds). It was noted under fluoroscopy that the distal end of the stent never exited the guiding catheter and so the entire fractured sds was removed together with the guide catheter. The procedure was completed with another of the same device. No patient harm nor complications were reported.
