Investigation evaluation: a review of the complaint history, device history record, dimensional verification, drawing, instructions for use (ifu), manufacturing instructions, quality control, and specifications of the device, as well as a visual inspection and functional test, were conducted during the investigation. The visual inspection of the point of separation the braided shaft is twisted and tapered which suggest torque was applied the device. The device was separated at the hub and shaft material in the hub confirms the device experienced shaft separation and not hub separation as originally reported.Damage to the shaft was present on the distal portion which most likely was caused by the reported vessel anatomy which included calcification and tortuosity.Additionally, a document-based investigation evaluation was performed of lot number 9248983 was reviewed, and no non-conformances were noted on the device lot.A software search revealed no other complaints associated to the lot number 9248983.Based on the information provided, and the results of the investigation, cause could not be traced to device. The appropriate personnel have been notified.Per the quality engineering risk assessment, no further action is required.Cook will continue to monitor for similar complaints. This report is required by the fda under 21 cfr part 803 and is based on unconfirmed information submitted by others.Neither the submission of this report nor any statement contained herein is intended to be an admission that any cook device is defective or malfunctioned, that a death or serious injury occurred, nor that any cook device caused, contributed to, or is likely to cause or contribute to a death or serious injury if a malfunction occurred.
