Event or Problem Description
The customer reported via phone call that the lip ring of the reservoir compartment was unable to lock in place when inserted into the insulin pump.The customer¿s blood glucose level was unknown.During troubleshooting, the customer stated that the reservoir lip ring was cracked and missing pieces of plastic.The customer was advised to discontinue use of the insulin pump and to revert to the back-up plan.The insulin pump will be returned for analysis.

Insulin pump received with reservoir tube lip completely broken off noted.The test reservoir does not stay locked in place due to reservoir tube lip broken off.Unable to perform displacement test due to reservoir lip broken off.
