The pump was returned, and analysis found overinfusion with an undetermined root cause.Interrogation of the device determined it was used to infuse water 1000mcg/ml for a total dose of 6.3 mcg/day.If information is provided in the future, a supplemental report will be issued.
 
Event Description
Information was provided by a manufacturer¿s representative regarding an implantable intrathecal pump.The indication for use was intractable spasticity and multiple sclerosis.The pump was returned to the manufacturer without a complaint.No patient symptoms or complications were reported as a result of this event.

Event Description
Additional information was received from the patient's healthcare provider via a clinical study on 2019-jan-04.It was reported that the patient's pump was implanted in (b)(6) 2012 and was explanted in (b)(6) 2018.The replacement was prophylactic as the pump was nearing end of life with 9 months from elective replacement indicator alarming.No further complications were reported or anticipated.
