Additional Manufacturer Narrative
Device analysis by mfr: returned product consisted of an innova self-expanding stent system with the thumbwheel protector still in the manufacturer position. The outer sheath, tip, inner sheath and the remainder of the device were checked for damage.Visual examination revealed kinks on the sheath at the nosecone, 33.9cm and 72.3cm from the nosecone. There is a kink on the middle sheath 3cm from the distal end of the middle sheath.Microscopic examination revealed that there is buckling on the middle sheath 2mm from the distal end of the middle sheath. The distal end of the middle sheath is damaged, and the tip is damaged. The stent did not return with the device. There is blood present inside the middle sheath. The device was functionally tested by inserting a 0.035" guidewire into a 6f introducer sheath. The device was then tracked along the guidewire thru the introducer sheath and there was some slight resistance felt from the buckling.
