Analysis of the ins (serial no.(b)(4)) found that there was abnormal function of an integrated circuit on the hybrid circuit board.If information is provided in the future, a supplemental report will be issued.
 
Event Description
Information was received from a healthcare provider (hcp) of a clinical study regarding a patient with an implantable neurostimulator (ins).It was reported that the hcp tried to retrieve the sensing data from the patient's device.The hcp programmed the sensing parameters to record on demand, and set the device to use an external trigger to start and stop recording.They turned the sensing off after updating the ins parameters.When they went into the "rec management" tab and clicked the "preview radio" then "execute" the error message "tx 235" was displayed.They were able to do a "send ext marker" and it seemed to do a recording, however they were unable to retrieve the recording.The data saved correctly after doing a "retrieve raw".The memory was cleared but this did not help resolve the error.It was noted they had another device configured the same way and were not experiencing any issues with that.No issues with stimulation functionality were observed.The root cause was not known.Additional information received from a manufacturing representative (rep) indicated that the device was explanted and replaced with a new device.The issue was resolved.
