Event or Problem Description
During an endoscopic retrograde cholangiopancreatography (ercp), the physician used a cook tri-tome pc triple lumen sphincterotome.[the user] by archering [bowing] the sphincterotome, the same curved at nine o'clock [incorrect cutting wire orientation], [making it] impossible for catheterization of the biliary duct.A section of the device did not remain inside the patient¿s body.The patient did not require any additional procedures due to this occurrence.According to the initial reporter, the patient did not experience any adverse effects due to this occurrence.
