Reported event: an event regarding abnormal ion level involving a metal head was reported. The event was not confirmed.Method & results: product evaluation and results: material analysis, visual, functional and dimensional inspections could not be performed as the device was not returned.Clinician review: a review of the provided medical records by a clinical consultant indicated: conclusion of assessment: this product inquiry concerns a male patient who underwent a primary left total hip arthroplasty that subsequently failed and approximately seven years later a revision was carried out due to elevated levels of cobalt and chromium.I can preliminarily confirm that these procedures occurred only through notations on implant sheets on the above dates in 2010 and then 2017.I could confirm with certainty if i had the operation notes available or primary and revision x-rays or office notes. The root cause of this event cannot be determined with certainty.Causes of elevated cobalt and chromium levels in the blood are multifactorial. These ion levels could be elevated due to erosion at the trunnion/head assembly or due to exogenous factors.Other factors include surgical technique factors, especially in the preparation of the trunnion and insertion of the femoral head, patient factors including bmi and activity level, and implant factors. The explanted prostheses should be submitted to stryker engineers for metallurgical analysis and evaluation.Product history review: review of the device history records indicate devices were manufactured and accepted into final stock with no relevant reported discrepancies.Complaint history review: there have been no other similar events for the lot referenced. Conclusions: it was reported that the patient was revised due to elevated metal ion levels. The exact cause of the event could not be determined because insufficient information was provided.Further information such as return of the device, pathology reports, pre- and post-operative x-rays and the primary operative report as well as patient history and follow-up notes are needed to complete the investigation for determining root cause. No further investigation for this event is possible at this time.If devices and/or additional information become available to indicate further evaluation is warranted, this record will be reopened.
