Device evaluated by mfr.: returned product consisted of a nc quantum apex balloon catheter. The outer shaft, inner shaft, balloon and tip were visually and microscopically examined.Visual examination revealed multiple minor kinks along the hypotube.Microscopic examination revealed that the guidewire lumen is buckled 47mm from the tip and is approximately 2mm long. There is a hole in the guidewire lumen in the buckled section approximately 48mm from the tip.The balloon is still tightly folded. The guide wire used in the clinical procedure was not returned with the device so a test guide wire was used for functional testing.A.014 in test guide wire was advanced into the guide wire lumen and was unable to pass the buckled section.Inspection of the remainder of the device presented no other damage or irregularities.
