Event or Problem Description
The customer reported via phone call that the insulin pump had piece was missing on the reservoir compartment and reservoir was unable to lock in place when inserted into pump because surrounds the reservoir was missing.The customer¿s blood glucose level was 85 mg/dl at the time of the incident.The customer was advised to discontinue use of the insulin pump and to revert to the back-up plan.The insulin pump will be returned for analysis.

Insulin pump received with reservoir tube lip completely broken off noted.The test reservoir does not stay locked in place due to reservoir tube lip broken off.Unable to perform displacement test due to reservoir lip broken off.
