Additional Manufacturer Narrative
|
|
The reported event that locking screw, fully threaded t2 tibia ø5x50 mm was alleged to be broken was confirmed, since the device was returned for evaluation and matches the alleged failure mode.Visual inspection of the returned product was performed, and the reported locking screw was found to be completely broken approx.Distal into two fragments each. All breakage surfaces of broken fragments show features of a fatigue fracture such as smooth topography and lines of rest.Plastic deformation was not found. The thread flanks of the locking screw are significantly flattened.Damage found at the threads of the screw such as friction signs resp.Bearing points indicate high axial load application.A review of the device history for the reported lot did not indicate any abnormalities. No corrective actions are required at this time.No indications of material, manufacturing or design related problems were found during the investigation.Based on investigation, the root cause was attributed to a patient related issue. The damage of the reported locking screw was most likely caused as a result of a fatigue fracture caused by repeated overloading during the implantation period.Overload may have been caused by patient¿s load application possibly in conjunction with impaired bone healing.Review of complaint history, capa databases, risk analysis and labelling did not identify any discrepancies. There are no open actions in place related to the reported event for the subject product(s).No non-conformity was identified. If any further information is provided, the complaint report will be updated.
