Additional Manufacturer Narrative
The following were reviewed as part of this investigation: patient severity, trend analysis, applicable previous investigation(s), sample (if available), applicable fmea documents, labeling, and applicable manufacture records.Based on a review of this information, the following was concluded: the complaint of guidewire becoming stuck within the catheter was confirmed and the cause appeared to be use-related.The product returned for evaluation was one 18ga x 2.25¿ accucath peripheral iv catheter.Usage residues were observed throughout the sample. The safety button was depressed and the needle was fully withdrawn into the housing. The catheter overlaid the guidewire. The catheter exhibited abundant bends and kinks along its entire length. The tip of the wire appeared caught at the distal end of the catheter.Microscopic inspection of the sample revealed the guidewire to be intact. The coils were misaligned. The coil region appeared caught at the deformed distal end of the catheter. The tip of the catheter was crumpled with inward curving edges.Blood residue was abundant throughout the catheter. The crumpled catheter tip appeared to have contributed to the entanglement of the guidewire coils.Subsequent attempted guidewire retraction appeared to have further buckled the catheter.Such catheter damage can occur if advancement is attempted into tissue and through a tortuous path.A lot history review (lhr) of recx2906 showed one other similar product complaint(s) from this lot number.
