Additional Manufacturer Narrative
The reported event could be confirmed, since x-rays confirming the failure mode were provided.More detailed information about the patient's data and patient's activity level is needed in order to determine the root cause of the complaint. However, based on the past complaint history a possible cause for the implant loosening is, but is not limited to the application of inappropriate loads on the device could be one of the most probable reasons of the loosening. As a reminder, the ifu clearly states: "warnings (see the patient counseling information section also) 1.Strenuous loading, excessive mobility, and articular instability all may lead to accelerated wear and eventual failure by loosening, fracture, or dislocation of the device." a review of the device history was not possible because the lot number communicated was wrong. The reported lot number (10192-l06) does not yield any results on our lot number databases. We conclude that the lot provided is wrong. No corrective actions are required at this time.A review of the labeling did not indicate any abnormalities. No indications of material, manufacturing or design related problems were found during the investigation. If the device is returned or if any additional information is provided, the investigation will be reassessed.
