It was reported, a male of unknown age was undergoing a lower extremity angiogram procedure utilizing a crosscath support catheter. The patient's anatomy was accessed via the left groin for a target lesion in the right common iliac. The physician was going up and over the aortic bifurcation, when a chronic total occlusion (cto) of the right common iliac artery (rcia) was encountered.A guide wire (cook) was advanced through the cto and the complaint crosscath support catheter was tracked through the occlusion. The catheter was advanced to the right common femoral artery (cfa). The wire was removed and an angiogram was taken to confirm the lumen. The guide wire was advanced back through the complaint crosscath support catheter to attempt catheter removal.Significant resistance was encountered. The guide wire and catheter were safely removed together as a unit.A section of the device did not remain inside the patient¿s body. The patient did not require any additional procedures due to this occurrence.According to the initial reporter, the patient did not experience any adverse effects due to this occurrence. This device was returned to the manufacturer and, upon preliminary visual inspection, was found to be separated and elongated.

B5: as reported, the patient had "very" calcified anatomy.A review of the complaint history, device history record, drawing, instructions for use, manufacturing instructions, quality control, and a visual inspection of the returned device were conducted during the investigation. The visual inspection of the returned device confirmed elongation and separation of the device. The device was returned in two elongated pieces; the proximal section measured 85.4 centimeters (cm) and the distal section measured 34.8 cm.Additionally, a document-based investigation evaluation was performed. There is no evidence to suggest the product was not made to specifications.A review of the device history record showed no nonconforming events which could contribute to this failure mode.It should be noted there were no other reported complaints for this lot number. This device is packaged with instructions for use, which warn the user against advancement of the catheter through resistance unless the source of the resistance is identified by fluoroscopy and appropriate steps are taken to reduce or remove the obstruction. It is known that the user in this case encountered significant resistance upon removal of the device, and the patient had a chronic total occlusion of the right common iliac artery and calcified anatomy.Furthermore, a review of the manufactures instructions, drawing, and quality control procedures was conducted, and no gaps were discovered.Based on the information provided and the examination of the returned product, investigation has concluded that a definitive cause for this event could not be established. We will continue our monitoring of similar complaints and have notified the appropriate personnel of this event.Per the quality engineering risk assessment, no further action is required. This report is required by the fda under 21 cfr part 803 and is based on unconfirmed information submitted by others.Neither the submission of this report nor any statement contained herein is intended to be an admission that any cook device is defective or malfunctioned, that a death or serious injury occurred, nor that any cook device caused, contributed to, or is likely to cause or contribute to a death or serious injury if a malfunction occurred.
