Event or Problem Description
The patient was initially implanted with an afx2 bifurcated stent graft to treat an abdominal aortic aneurysm.Approximately two (2) months post initial implant the patient presented with stent occlusion. It was stated that the patient would require above the knee amputation due to the occluded graft. The patient was also found to have multiple organ failure (renal failure and respiratory failure) but the cause could not be determined due to the patient being a chronic drug user. The patients family has elected not to perform any interventions but rather have the patient placed in palliative care.No further additional information or patient sequalae has been reported at this time.

Additional Manufacturer Narrative
The manufacturing lot evaluation confirmed all devices met specifications prior to release. The device was not returned as the device remains implanted therefore no evaluation was completed.A clinical assessment of the reported event could not be completed due to a lack of receipt of relevant medical records and/or imaging. An attempt was made to obtain medical records and/or images but was denied. As such, event determination, off label conditions, related patient harms and patient disposition could not be independently assessed. However; as it was reported that the patient was a chronic drug user, this is a contributing factor for this event. No additional investigation of this reported event is planned however if additional information pertinent to the incident is obtained, a follow-up report will be submitted.Endologix will continue to monitor this complaint and similar complaints in the event further investigation is needed.Device iteration is afx2.

Event or Problem Description
Additional information: this patient had a previously reported event for thrombus that was identified throughout the afx2 device.Thrombectomy was performed in the aorta and common iliacs (bilateral) and the patient's left leg was amputated below the knee due to the thrombosis. This event was reported under 2031527-2019-00096.
