During the procedure an attempt was made to use one resolute onyx rx coronary drug eluting stent to treat a non-tortuous and mildly calcified lesion located in the ramus with 100% cto.The device was inspected with no issues noted.Negative prep was performed with no issues. The lesion was pre-dilated.The device did pass through a previously-deployed stent.Resistance was encountered when advancing the device.Excessive force was used during delivery. It was reported that stent dislodgement occurred after removal from the patient. It was indicted that the stent was too long and a different size stent was preferred. The stent was therefore removed and on removal the stent felt loose.
