Additional Manufacturer Narrative
Patient code 2001 captures the reportable issue of perforation.Investigation results: a visual examination of the returned complaint device did not show visual defects and the balloon bonds were inspected and found to be continuous and intact.Additionally, the distal tip of the device was carefully inspected and it looked normal, no deformations nor damages were observed.A functional examination was performed and the balloon was able to be inflated up to its two recommended inflation volumes; there were no leaks, holes, pinholes noted and the balloon was able to hold the pressure.A dimensional examination was performed and the balloon's outer diameter (od) was measured at its two recommended volumes; low and high passed and were within specification.Although there was no alleged malfunction of the device; however, the tip of the device caused a perforation in the biliary tract and perforation is noted within the dfu as a potential adverse events associated with the use of this device. Therefore, the most probable root cause is known inherent risk of device.A review of the device history record (dhr) was performed and confirmed that this device met all material, assembly and performance specifications.A labeling review was performed and from the information available, this device was used per the directions for use (dfu)/product label.
